As part of our growing team, we’re expanding our relief pool – an ideal opportunity for anyone looking for flexible work that fits around their life. Whether you\’re a student, a parent, semi-retired, or simply after a better work-life balance, joining our relief team on a zero-hour contract lets you choose when you work. We’re looking for Relief Support Practitioners to join our teams in Perth and Bridge of Earn to support people across our Housing Support services.

Our Housing Support Services providing support 24 hours a day all year round. Shift patterns vary and include early and late shifts, evenings, weekends and public holidays – so there is something to suit everyone.

No two days are the same. You’ll work alongside a dedicated team, providing person-centred support to people with a range of complex needs. Many of the people we support don’t use verbal communication, so creativity, patience and empathy are essential.

At the heart of what we do is value-based communication – adapting how we connect and support each person in ways that work for them. Our teams help people live full, independent lives, making their own choices and building their confidence every day.

Responsibilities of the Support Practitioner will include:

  • Healthcare Support: Assist with healthcare needs, including medication administration and personal care, ensuring the well-being of individuals.
  • Record Keeping: Maintain accurate and up-to-date records of daily tasks and activities, ensuring clear documentation of progress and support provided.
  • Collaborative Care: Work closely with colleagues, families, and individuals to deliver high-quality care and support, fostering a team-oriented approach.
  • Encouraging Independence: Promote independence and personal growth by following Support Plans tailored to each individual\’s needs and goals. This could include supporting them with everyday tasks or activities they enjoy, such as going shopping, attending community groups, enjoying walks, or taking part in hobbies and outings that matter to them.
