Young People Leaving Care Support Worker – Preston Area – East Lancs
Position Overview: This outreach role involves driving to meet young people for appointments. The office is based in Preston, covering a broader area including Wigan. A valid UK driving license with business insurance and access to your own car are essential.
Contract Details: Immediate start on a long-term temporary contract, with the possibility of becoming permanent after 12 weeks. The contract will last at least 3 months.
Compensation: £15.50 per hour plus 40p mileage.

Experience & Qualifications:

Main Duties & Responsibilities:
- Assist young people\’s social workers in service delivery, including:
- Providing transport and escorts as per care plans.
- Conducting welcome and induction sessions.
- Assessing and supporting service users\’ needs.
- Supporting welfare and NHS issues.
- Preparing reports and arranging appointments with partner agencies.
- Maintaining contact with service users and informing them of support groups.
- Supporting move-on processes.
