Semester Recruitment is seeking a Support Worker for Wrexham County Borough Council, providing high-quality, person-centred home care and reablement support to vulnerable adults within their own homes.
Working across a caseload that includes older people and individuals with physical and sensory impairments, you will support service users to live as independently as possible. Key responsibilities include delivering personal care (washing, dressing, toileting, bathing), prompting medication, managing catheter care, assisting with food preparation and feeding, and supporting mobility using appropriate equipment.
You will also respond to emergencies including falls assessment, liaise with managers, and contribute to the development and review of individual care plans and health action plans.
The role requires building and maintaining professional relationships with service users, their families, and carers, encouraging social activities and independence through reablement and intermediate care approaches.
This is a day-rate contract position paying £98.12–£129.15 per day, starting 31 August 2026, sourced via Prism. Candidates must be eligible to work in Wales and able to attend the Wrexham area.
