Young People Leaving Care Outreach Support Worker.
Covering Leeds, full time, 37.5 hours a week.
Outreach – hours are between 9am and 5 pm, between Monday To Friday.
Temporary positon
£17+ an hour plus mileage covered.
Mileage is paid at 55p per mile.
Essential Criteria:
- Experience supporting young people and their families
- Full Uk Driving License is essential with Business Insurance
- Completing property inspections
- Supporting young people to develop independence skills
- Managing their diary and working closely with permanent staff
- Completing detailed reports
Our client is a leading Provider of care to children and young people in the area, a quick start is possible if you have a current online DBS
It is essential that you have experience of working within a similar setting and preferably have completed NVQ3 Children & Young People Workforce or a similar qualification within this sector.
Qualifications/ Requirements
- High standard of general education.
- Preferable: NVQ Level 3 Health & Social Care – Children and young People (CCYP) or equivalent.
- Willingness to undertake further training.
Job Purpose
To be responsible for the provision of Support for Vulnerable People (Service Users) for the Company.
Main Duties & Responsibilities
- To assist the Young person’s allocated Social Worker in the delivery of the service.
- Provide transport/escort to the young person as directed in line with care plans
- Conduct welcome and induction for Young people
- Ongoing assessments of Service Users’ needs
- Provide ongoing support and monitoring of Service Users
- Assist in welfare and NHS issues
- Preparation of reports
- Arrange appointments with various partner agencies
- Maintaining regular contact with Service Users
- Informing Service Users of support groups
- Assist Move-On of Service Users
- To accurately record, create, maintain and update electronic records in respect of our Service Users.
- To prepare for and attend meetings as required or directed.
- To provide mediation to Service Users as required and/or directed.
- To effectively use IT packages to support the work of the company.
- To work effectively and cooperatively across multi disciplinary teams.
- To observe and maintain strict confidentiality in respect of the Service Users, Clients and Company.
- To pro-actively assist in the development of policies and procedures of the Company.
- To report any housing issues to the Services Department and to assist, where required, in the resolution of any defects.
- To undertake On-Call duties as directed.
- To participate, where required, in office reception duties.
