The Adult Social Care Review Team covers a wide remit of reviews and reassessment across the Wirral, working with persons recently discharged from hospital and those residing in the community. The Review Team supports various adult social care projects and at-risk care home and domiciliary providers. It collaborates with Wirral contracts & commissioning and quality improvement teams, adhering to outcomes-focused and legal literate practices. The team offers opportunities for practitioners seeking to strengthen their practice and apply transferable knowledge and skills in a CPD-focused environment. Agency Social Workers are required to support service delivery, utilizing assessment and intervention skills within a legal framework. Experience working with older adults is essential.

- Degree in Social Work
- SWE Registration
- Right to work within the UK
