Social worker required for Hospital Discharge Neighbourhood Team based in the West/North West area of Leeds. Managing discharges from out of area hospital, recovery hubs, temporary placements and reviewing care in people’s own homes.
Role is ICB funded and pays £35ph – role is required for 12 weeks. Knowledge of Leeds CIS Apex recording system is essential.
You must have:
- A professional qualification in social work
- Social Work England registration
- Enhanced DBS/Update service
- References required for the last 5 years of employment
