The successful candidate will work autonomously and collaboratively within a multi-disciplinary team to assess, diagnose, treat, and manage patients with complex symptoms that have not yet been diagnosed.
Role Duties
- Clinical Practice; independently assess, examine, diagnose, and manage patients presenting with acute illness and acute exacerbations of chronic disease.
Clinical Responsibilities
- Provide expert management of conditions including COPD and asthma exacerbations, heart failure decompensation, diabetes complications, frailty and complex multimorbidity, acute infections in patients with chronic disease, chronic pain flare ups, hypertension and cardiovascular instability.
- Request, interpret, and act upon diagnostic investigations.
- Prescribe medications independently within scope of practice.
- Develop and implement evidence-based management plans.
- Identify deteriorating patients and escalate appropriately.
- Deliver same day urgent care clinics and home visit assessments where required.
Qualifications
- Registered healthcare professional (NMC/HCPC)
- MSc Advanced Clinical Practice (or working towards)
- Independent Prescriber qualification
Skills
- Strong clinical assessment and diagnostic skills
- Ability to work autonomously and manage clinical risk effectively
- Strong communication and multidisciplinary team‑working skills
- Non‑medical prescribing experience, including managing patients with complex co‑morbidities
- Understanding of NHS frameworks and primary care pathways
Experience
- Significant experience in primary care, urgent care, emergency care or community services
- Demonstrable experience in managing acute and chronic conditions
- Experience working in a GP practice or Primary Care Network
- Experience in frailty, respiratory, diabetes or cardiovascular care
- Experience in teaching, mentoring or leadership

We are currently a GP partnership of 30 practices with 38 surgeries, serving around 280,000 patients in Birmingham, Wolverhampton, and Shropshire. Our head office is based in Cotteridge, but this role will be based at Selly Oak Health Centre.
