We are hiring for Community Occupational Therapists Band 6 Based at Alsager Medical Centre but working across Sandbach, Middlewich, Alsager, Scholar Green and Haslington areas Job Purpose / Main Duties / Responsibilities The Post holder will: · Be competent as appropriate to the requirements of the role. · Putting people at the heart of what we do, Empowering People to improve Care and Wellbeing, Delivering better Health, better Care in partnership. · Be a Specialist Occupational Therapist and have a detailed professional and clinical knowledge. They will be a specialist practitioner and have a focus in one of the following areas; Rehabilitation, Integrated Community, Community Intervention and Reablement. · Be responsible for a defined population of service users in a variety of settings across the Partnership Trust including the community and hospitals. · Be skilled in the assessment of complex/specialist care/support needs and in the development, implementation and evaluation of care/support packages. · Have supervisory, training and leadership responsibilities where appropriate and will have some responsibility for team performance and service developments/improvements. · Demonstrate initiative and be creative in finding solutions to problems. · Work with the team leader undertaking delegated tasks in team management. Qualifications- Degree in Occupational Therapy or equivalent. Registration with HCPC Fieldwork educator’s accreditation. Training Post qualification courses in working with older people and people with physical disabilities. Attendance at relevant clinical networks or Special Interest Groups Up to date field work/pre-registration student training Work Experience Relevant experience to the role including assessment and interventions for a wide range of disabilities. Ability to manage time and caseload effectively. Supporting service users Car Drivers only. Min 6 months NHS experience mandatory
