- We are seeking experienced and motivated
- Advanced Clinical Practitioners (ACPs) to join our established
South East London Out of Hours (SE OOH) service .
As an ACP, you will work autonomously as part of a supportive multidisciplinary team, delivering high-quality urgent care through telephone triage, face-to-face consultations and home visits.
This is an excellent opportunity for clinicians looking for flexible sessional work within one of South London's longest-established GP cooperatives.
Key Responsibilities
- Provide autonomous clinical care to patients of all ages presenting with undifferentiated urgent care conditions.
- Deliver care through telephone triage, face-to-face consultations and home visits, according to service requirements.
- Undertake comprehensive history-taking, clinical examination and patient assessment.
- Use advanced clinical reasoning to assess, diagnose and manage patients safely and effectively.
- Develop patient-centred management plans, including appropriate prescribing, referrals and safety-netting advice.
- Communicate effectively within the multidisciplinary team to support high-quality patient care.
- Maintain accurate clinical records using healthcare technology to support clinical decision-making.
- Contribute to service improvement initiatives while maintaining the highest standards of patient care.

Person Specification
Essential
- Current registration with the NMC or HCPC.
- Non-Medical Prescribing qualification.
- Postgraduate Qualification in Advanced Practice.
- Advanced Clinical Practitioner MSc qualification (Level 7), or working towards completion.
- Demonstrable experience in autonomous practice within Primary Care, Urgent Care, Emergency Department or other first-contact settings.
- Evidence of comprehensive history-taking, clinical examination and clinical decision-making skills.
- Experience in paediatrics, including children under the age of two years.
- Strong clinical reasoning, problem-solving and communication skills.
