The opportunity

Deliver acute clinical care where it can make a profound difference in patients own homes.

SELDOC is an established GP co-operative with more than 30 years experience delivering healthcare services across London. We are seeking experienced GPs to join our @home service.

This is more than a traditional home-visiting role.

You will assess, treat and coordinate care for acutely unwell patients who might otherwise require hospital admission or a longer inpatient stay.

The role

Working within a highly skilled multidisciplinary team, you will:

• Assess and treat acutely unwell patients at home
• Review new referrals and higher-acuity patients
• Participate in virtual ward rounds
• Review results, medicines and treatment plans
• Arrange investigations, follow-up care or hospital escalation
• Complete medicines reconciliation, clinical records and electronic discharge letters

Presentations include cellulitis, COPD, dehydration, heart failure, infections, falls, frailty, post-operative needs and general deterioration.

About you

You must be a GMC-registered GP on the NHS Medical Performers List, with appropriate indemnity and the right to work in the UK.

You must be confident in:

• Managing acutely unwell patients
• Cannulation and venepuncture
• Administering IV antibiotics and IV/subcutaneous fluids
• Male and female catheterisation
• ECG recording and interpretation
• Wound care
• NEWS2, Waterlow and MUST scoring

Bladder scanning and experience managing COPD, heart failure and common geriatric conditions are desirable.

Successful candidates must complete an @home GP Skills Declaration.
