Selby Town PCN is seeking a Clinical Pharmacist to join a multi-disciplinary team in a primary care setting.

You will review chronic disease management, optimise medicines, and support the Direct Enhanced Service PCN prescribing agenda with structured medication reviews.

You will work closely with GP teams, care homes, and other professionals to sustain high-quality patient care, improve safety, and reduce medicine waste.
