Sefton Council is seeking an Adult Social Worker to join the South Community Team. You will manage a complex caseload of adults with care and support needs, conducting assessments, reviews and support planning in line with the Care Act 2014 and other legislation.
You will work with individuals, families and partners to develop personalised support plans that promote independence, wellbeing, and positive outcomes. This is a challenging and rewarding role within a supportive team.
