Sefton Metropolitan Borough Council is recruiting a Social Worker for Adults with Learning Disabilities and/or Autism to deliver person-centred assessments, care planning and reviews under the Care Act. You will work within Adult Social Care, supported by lead practitioners and managers, managing a varied caseload, applying MCA and safeguarding principles, and collaborating with health colleagues to promote independence and wellbeing.
