Care Team Leader

Job Overview

We are seeking an experienced Care Team Leader to support our growing home care team. In this role, you will split your time between delivering high-quality person-centered care in clients' homes and conducting field supervisions, care reviews, and staff mentoring in the community.

Key Responsibilities

  • Field Supervision & Support: Conduct spot-checks, competency assessments, and shadow training for new and existing Care Assistants in the field.
  • Care Assessment & Planning: Carry out initial client assessments, write detailed digital care plans, and review risk assessments as clients' needs evolve.
  • Quality Assurance: Ensure all home visits meet CQC compliance standards, audit medication MAR charts, and review daily log notes.
  • On-Call Support: Participate in the shared out-of-hours emergency on-call rota to assist field staff and manage roster adjustments.
  • Direct Care: Cover client visits in emergency situations or staff absences to guarantee continuity of care.

About the role

Requirements & Qualifications

  • License & Transport: Full UK Driving License (Essential).
  • Experience: Minimum 2 years in Domiciliary/Community Care, with at least 1 year in a senior or supervisory capacity(Preferred)
  • Compliance: Enhanced DBS on the Update Service (or willingness to undertake one).
  • Preferred Skills: Proficiency with digital care planning apps (e.g., Access Care Planning).
