Reports to: Care Manager

Summary of role: Ensure all client visits are allocated, recruit suitable Care and Support Workers, and maintain appropriate office procedures.

About the role: Help Caremark on its mission to fixing the way we provide care! You will require a UK driving license and access to a car. Our award-winning team in Dartford, Gravesham and Medway are really passionate about care and making sure everyone at our service receives the highest quality of care in their own homes. We are now looking for another kind, caring individual to join our team and help us on our mission to providing the best possible care across Dartford, Gravesham and Medway.

What the Care Coordinator role involves:

  • Organisation of rotas by liaising with our care team
  • Managing new customer referrals, following up enquiries and converting opportunities into new care packages
  • Onboarding new customers and ensuring care is set up smoothly from the start
  • Liaising with customers, families and care staff to ensure care needs are met and changes are responded to promptly
  • Maintaining accurate care records and ensuring information is kept up to date
  • Providing exceptional customer service for clients and care teams
  • Assist with on call duties

  • You should have proficient IT skills as rotas and care plans are updated using our IT systems
  • A team player

The role is based in our Medway office and is a full-time position.
