Praxis Care is seeking a Team Leader on a 3-month fixed term, 32 hours per week, to lead a team supporting individuals with complex needs. You will design individual support plans, coordinate delivery by Support Workers and yourself, and ensure care complies with policy and statutory requirements.
Essential: 2 years in health and social care, leadership or management qualifications, and strong communication skills. Desirable: experience in mental health or learning disability services.
