Do you enjoy the challenge of working with clients experiencing multiple disadvantages? Are you a holistic and dynamic practitioner? Are you interested in joining a small team embedded within a multi-disciplinary partnership primary care service for individuals experiencing homelessness?

We are seeking a motivated individual who can demonstrate the skills required to develop trusting relationships and support individuals experiencing multiple disadvantages and facing significant systemic barriers to accessing and engaging with support and healthcare. The successful candidates will be working within the Health Link Team alongside partner teams within the Homeless Health Service as well as a wide range of external agencies coordinating tailored support, identifying creative solutions, and empowering individuals to access and engage with health services more independently.

The Health Link team works with people who are homeless, at risk of repeat homelessness or who are vulnerably housed in order to improve their health, recovery and maximise engagement with healthcare and related services. The team specialises in working with people who experience chronic, complex health needs and multiple disadvantages, and who face systemic challenges which impact on engagement with health services. Health Link works with individuals who are frequent attenders of A&E services.
