We are recruiting Recovery Navigators to join our Intensive Outreach Service Team in South Wiltshire, Swindon, and BANES to provide personalised support to individuals who experience complex needs associated with multiple and severe disadvantage. The Intensive Outreach Service (IOS) works across Bath, Northeast Somerset, Wiltshire and Swindon, supporting individuals with intensive mental health symptoms or crisis who require additional intervention beyond short‑term clinical input.
Responsibilities
- Work as part of an integrated system alongside AWP Intensive Services, contributing to the delivery and continuous development of the IOS model through collaborative practice and reflective learning within agreed role boundaries.
- Conduct person‑centred support conversations to assess what matters to each client, establish goals, and use the Dialog+ tool to monitor outcomes.
- Provide intensive, trauma‑informed, face‑to‑face support, responding flexibly to acute mental health distress, crisis presentations and fluctuating levels of risk, including stabilisation, grounding techniques, crisis management, effective coping skills and safety planning.
- Assist clients to navigate the wider system through signposting or direct support in accessing housing, debt advice, health and community resources, employment, and family or relationship support.
- Manage a complex caseload, maintaining accurate, clinically informed records that detail support engagement and risk observations, while recognising the distinction between factual information and professional opinion.

- Experience working with people with mental health needs, including acute distress, crisis, and co‑occurring needs.
- Strong knowledge of mental health presentations and experience with support planning and risk assessment.
- Commitment to trauma‑informed, psychologically informed and recovery‑oriented practice.
- Passion for improving the lived experience and health of people using mental health services.
- Knowledge of the needs of people with mental health challenges and proven ability to provide tailored support.
