Main duties of the job:
- This is a support role helping older adults often with complex needs, like mental health, housing, substance abuse, fragility and social isolation, connect with services and build independence.
- The Recovery Navigator acts as a key link between clinical care and community support to improve well-being and achieve personal goals, focusing on the unique challenges of aging.

Job responsibilities:
- Working with families, carers and friends and be working in a multidisciplinary team.
- Building therapeutic relationships with service users.
- Support a caseload of severe and enduring mental health needs of service users.
- An excellent communicator.
- Receiving management and clinical supervision, together with group reflective practice.
Person Specification:
Experience:
- Proven experience of working with people with multiple needs (e.g. mental health, homelessness, alcohol/drug dependency long term conditions).
- Two years proven experience working with clients over the age of 50 in a community setting.
- Proven experience of carrying out needs and/or risk assessments and the support planning process.
- Experience of working with a number of individuals with competing needs and priorities.
- A year’s experience.
