This is an exciting opportunity to work across a broad range of nursing services, including long-term condition management, wound care, health promotion and our recently launched Minor Injuries Service. No two days are the same, making this an ideal role for someone who enjoys variety and values teamwork.

We welcome applications from nurses with experience in general practice, community nursing, district nursing, urgent care, outpatient services or other relevant clinical settings. Experience in diabetes, wound care or minor injuries is desirable but not essential, as training and development will be supported.

Main duties of the job

As a Registered Nurse, you will play a key role in delivering high-quality, patient-centred care across a varied clinical workload within our practice.

Your responsibilities will include long-term condition management, wound care and dressings, health promotion, immunisations and vaccinations, cervical screening (if qualified), and supporting patients with both acute and routine nursing needs.

You will also contribute to our recently launched Minor Injuries Service, assessing and treating patients within your scope of practice and supporting GPs with unexpected presentations when required.

Working closely with our multidisciplinary team, you will provide evidence-based care, promote healthy lifestyles, maintain accurate clinical records and contribute to the ongoing development of our nursing services.

Job Purpose

To provide high-quality, evidence-based nursing care to patients registered with Seaton & Colyton Medical Practice, supporting the delivery of general practice nursing services. The post holder will work collaboratively within the multidisciplinary team to deliver safe, effective and patient-centred care across a varied clinical workload, including long-term condition management, wound care, health promotion and the Minor Injuries Service.

Main Duties and Responsibilities

The successful candidate will work within their scope of practice and competency to:

Clinical

• Assess, plan, implement and evaluate nursing care.
• Deliver evidence-based care to patients presenting with acute and chronic conditions.
• Undertake long-term condition reviews, including diabetes and other chronic diseases (within competency).
• Provide wound assessment, dressings and ongoing wound management.
• Assess and treat patients presenting to the Minor Injuries Service within their scope of practice.
• Support GPs with urgent or unexpected patient presentations when required.
• Deliver immunisations and vaccinations in accordance with national guidance.
• Undertake cervical screening where appropriately trained.
• Provide lifestyle advice and health promotion to support disease prevention and self-management.
• Carry out other clinical procedures appropriate to the role, including venepuncture, blood pressure monitoring and ECGs (where trained).
• Maintain accurate, contemporaneous electronic patient records using the practice clinical system.
• Ensure patient confidentiality is maintained at all times.

Professional Responsibilities

• Maintain current NMC registration and practise in accordance with the NMC Code.
• Maintain professional competence through continuing professional development.
• Participate in mandatory training, appraisal and clinical supervision.
• Work in accordance with practice policies, national guidance and evidence-based practice.
• Recognise and respond appropriately to safeguarding concerns.
• Adhere to infection prevention and control policies.

Team Working

• Work collaboratively with GPs, nurses, healthcare assistants, administrative staff and the wider multidisciplinary team.
• Contribute to service development and quality improvement initiatives.
• Support colleagues and promote a positive team culture.
• Participate in practice meetings and clinical audit where appropriate.

General

The post holder may be required to undertake other duties appropriate to the grade of the post, in keeping with the needs of the practice and within their competence.

Person Specification

Qualifications

• Registered Nurse with current NMC registration.
• Evidence of ongoing professional development and commitment to maintaining clinical skills.
• Ability and willingness to learn new skills
• Long-term condition management qualification (including diabetes).

Experience

• Experience of assessing, planning and delivering patient-centred care.
• Ability to work effectively as part of a multidisciplinary team.
