South Downs Health and Care GP Federation is recruiting two Frailty Practitioners to join the Integrated Neighbourhood Team in Eastbourne. You will perform advanced clinical assessments, develop personalised care plans and coordinate with GPs, geriatricians and community teams to support frail patients at home.
You will contribute to service development, governance and quality improvement, with independent prescribing and medication reviews within professional codes.
