As a Screen Clinical Pharmacist, you will utilise your skills as a medicines expert to help identify patients in need of pharmacological and non-pharmacological interventions in Primary Care. If you are looking for a career that is stimulating, challenging, and will enable you to support patients through medicines optimisation and patient review.
Delivering face-to-face asthma and COPD reviews, recommending evidence-based therapy adjustments in line with current clinical guidelines, with the opportunity to make a meaningful impact and enhance patients’ quality of life and health outcomes.
- Undertaking medicines optimisation reviews to ensure practices adhere to clinical governance standards and prescribe in accordance with the NI Formulary.
- Supporting and delivering a wide range of clinical services, including (but not limited to) self-monitoring of blood glucose, osteoporosis and vitamin D management, pen needle provision, and respiratory care (COPD and asthma).
