Social Worker - Learning Disabilities Team Location-West Midlands Duration - 12 weeks Hours per week- 37 hours Hybrid Working Pay Rate-28.40

The Role

As a Social Worker within the Learning Disabilities Team, you will manage a varied caseload and work closely with individuals, families, carers, health professionals, and partner agencies to achieve positive outcomes.

Key responsibilities include

  • Undertaking strengths-based assessments in line with the Care Act 2014.
  • Developing and reviewing person-centred care and support plans.
  • Supporting adults with learning disabilities to maximise independence and wellbeing.
  • Completing Mental Capacity Act assessments and Best Interest decisions where required.
  • Managing safeguarding enquiries and promoting positive risk-taking.
  • Working collaboratively with health colleagues, housing providers, advocacy services, and community organisations.
  • Supporting transitions, hospital discharges, and long-term support planning where appropriate.
  • Ensuring individuals have access to the right services, resources, and opportunities within their local communities.
  • Maintaining accurate, timely, and comprehensive case records.
  • Contributing to a culture of continuous improvement and high-quality practice.

About You

We are looking for a compassionate and skilled Social Worker who is committed to promoting rights, independence, and inclusion for adults with learning disabilities. You will:

  • Be confident managing complex cases and making informed professional decisions.
  • Demonstrate excellent assessment and care planning skills.
  • Have a strong understanding of strengths-based and person-centred practice.
  • Be an effective communicator with the ability to build positive relationships with individuals, families, and professionals.
  • Work collaboratively as part of a multidisciplinary team.
  • Be committed to achieving positive outcomes and improving quality of life for service users.

Essential Requirements

  • Qualified Social Worker (Degree, Dip SW, CQSW or equivalent).
  • Registration with Social Work England.
  • Experience working within Adult Social Care.
  • Sound knowledge of: Care Act 2014, Mental Capacity Act 2005, Safeguarding Adults Procedures, Human Rights Act, Equality and Disability legislation.
  • Strong assessment, risk management, and care planning skills.
  • Full UK driving licence and access to a vehicle (where required).

This role is exempt from the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975.

As this role is eligible for an Enhanced DBS check, applicants are required to declare any cautions and convictions that are not protected under current filtering rules.
