Social Worker - Hospital Discharge Team Location- West Midlands Duration-12 weeks Hours per week- 37 hours Pay rate- £28.40We are currently recruiting for an experienced Social Worker to join a busy and dedicated Hospital Discharge Team in the West Midlands.

This is an exciting opportunity to work at the heart of integrated health and social care services, supporting adults to achieve safe, timely, and person-centred discharges from hospital.

You will play a key role in promoting independence, reducing delays, and ensuring individuals receive the right support at the right time to maximise their recovery and wellbeing.

The Role

As a Hospital Social Worker, you will work closely with NHS colleagues, community services, individuals, and their families to facilitate effective discharge planning and prevent unnecessary hospital admissions and readmissions.

  • Undertaking strengths-based assessments to identify care and support needs.
  • Supporting safe and timely hospital discharges in line with discharge-to-assess principles.
  • Completing Care Act assessments and developing person-centred support plans.
  • Assessing and managing risk while promoting choice, control, and independence.
  • Working collaboratively with multidisciplinary professionals, including health colleagues, therapists, and community providers.
  • Completing Mental Capacity Act assessments and Best Interest decisions where required.
  • Providing advice, information, and support to individuals, families, and carers.
  • Arranging appropriate care packages, placements, and community support services.
  • Managing a varied caseload within a fast-paced hospital environment.
  • Maintaining accurate, timely, and comprehensive case records.
  • Ensuring safeguarding concerns are identified and responded to appropriately.

Essential Requirements

  • Qualified Social Worker (Degree, Dip SW, CQSW or equivalent).
  • Registered with Social Work England.
  • Experience working within Adult Social Care.

Sound knowledge of: Care Act 2014Mental Capacity Act 2005Safeguarding Adults Procedures Human Rights legislation

Strong assessment, risk management, and care planning skills.

This role is exempt from the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975.

As this role is eligible for an Enhanced DBS check, applicants are required to declare any cautions and convictions that are not protected under current filtering rules.
