Purpose of the Role
St Paul’s Clinic is seeking a dedicated Practice Nurse to deliver high-quality, evidence-based care in a vibrant city-centre practice. This role focuses on childhood immunisations, chronic disease management, and wound care, while supporting health promotion and patient education across a diverse population.
Key Responsibilities
Clinical Duties:
- Administer childhood immunisations and maintain accurate records
- Manage chronic conditions (diabetes, asthma, COPD, hypertension)
- Provide wound care, dressings, and post-operative follow-up
- Support health promotion (smoking cessation, weight management)
- Conduct cervical screening and contraceptive advice (if trained)
- Assist with ECGs, phlebotomy, ear syringing, injections, and minor illness assessments
- Support medication adherence and reviews (if qualified)
- Work closely with GPs, pharmacists, and healthcare assistants
- Participate in multidisciplinary meetings and audits
- Engage in CPD and maintain clinical competencies
- Contribute to quality improvement initiatives and policy updates
Administrative & Compliance:
- Maintain accurate records using EMIS
- Ensure compliance with NHS Wales, NICE, and Public Health Wales standards
- Support HIW inspections and regulatory requirements
Skills, Experience & Qualifications
- Experience in immunisations, chronic disease management, and wound care
- Strong communication and organisational skills
- Ability to work independently and within a team
- UK professional registration
- Willingness to undergo a DBS check
- Diploma in Diabetes or Respiratory Care
- Experience in women’s health, triage, and multilingual patient care
- Familiarity with NHS protocols and EMIS system
- Background in socioeconomically disadvantaged communities
