Care Plus Group is seeking a Health Care Assistant to join the Community Inpatient Unit (CIU) at Cambridge Park, Grimsby. This permanent, part-time role (24 hours/week) offers a fixed salary point of £24,968/year pro rata. The CIU provides short-term, bed-based rehabilitation, re-enablement, and recovery support for individuals following illness, injury, surgery, or significant health events. The successful candidate will deliver personal, nutritional, and clinical care under the supervision of Nurses and Assistant Practitioners, supporting patient independence and recovery.
Key Responsibilities
- Deliver personal and nutritional care to patients
- Undertake clinical tasks (observations, dressings, swabs, etc.) with expanding skills over time
- Coordinate direct care for a cohort of patients under supervision
- Work collaboratively within a multidisciplinary team (Nurses, Physiotherapists, Occupational Therapists, Social Workers, Care Staff)
- Maintain accurate records and uphold confidentiality
- Support patient-centred rehabilitation and recovery
Candidate Requirements
Essential:
- QCF Level 3 in Health and Social Care
- Level 2 qualification in Numeracy and Literacy (or equivalent competence)
- Experience working with patients
- Effective computer skills
- Ability to work independently and as part of a team
- Clear, sensitive communication skills
- Empathy, professionalism, and ability to handle challenging situations

- Flexible approach to working arrangements
- Disclosure and Barring Service (DBS) check
Desirable:
- Experience in multidisciplinary team working
- Experience using SystmOne clinical system
- Knowledge of community nursing and primary care
- Clinical skills such as wound care, venepuncture, blood pressure monitoring, and blood sugar monitoring
