As the Lead Nurse in Dementia, you will manage all aspects of clinical practice on the ward, ensuring adherence to evidence-based practice, local and national guidance and relevant professional standards. Providing support to the General Manager in day-to-day clinical management of the unit, the post holder will ensure a safe environment for all patients accessing the service. Promotion of the highest standards of care will be integral to the role, ensuring that co-ordinated packages of care are provided through effective and timely liaison with other health professionals.

Key Responsibilities:

  • Lead the Nursing and Healthcare team to ensure safe and effective nursing practices are embedded throughout Schoen Clinic York.
  • Work with the General Manager to ensure the rota is safely staffed to meet the needs of the service.
  • Participate in the on-call rota, managing day to day ward issues in line with Schoen Clinic policies.
  • Audit the on-call requirements with the nursing team and be aware and knowledgeable of when it is appropriate to escalate matters to the medical or management team.

Further duties will include (but not limited to):

Clinical

  • Ensure that the requirements of the Mental Health Act and Mental Health Capacity Act are fulfilled.
  • Ensure the full nursing team adhere to NMC guidelines.
  • Assist the Clinical Manager and medical staff with the highly specialist assessment, admission and discharge of patients.
  • Ensure that all colleagues follow the guidelines for the administration and safe keeping of drugs.
  • Ensure that all colleagues are familiar with, and adhere to, patients care plans.
  • Assess, plan, implement, and evaluate care, treatment and management of risk for individuals with complex needs, and support others to be able to do this.
  • Provide specialist advice which contributes to the care of patients.

Support

  • Provide support to colleagues in order to achieve targets.
  • Demonstrate effective leadership, being visible and accessible to patients, families/carers, and colleagues.
  • Operate within own professional competence and maintain professional boundaries.
  • Develop and, where appropriate, deliver training packages to enhance the knowledge and skills of the staff group and students.
  • Work with the senior leadership team to provide colleague development and training, thus ensuring colleagues are equipped to deliver the highest standards of care.
  • Represent the nursing team at meetings, educational events, and interface with Commissioners, families and carers of patients, as required.
  • Support the Clinical Manager in their managerial duties as required.

Administrative

  • Manage MDT meetings as required.
  • Manage the nursing team including supervision/appraisals, key working, etc.
  • Assist the Hospital Director in implementing appropriate policies and procedures e.g. clinical supervision, disciplinary, recruitment and selection, sickness and absence.
  • Undertake, support and oversee clinical audits e.g., patient notes, key and co-worker audits etc. Action any findings as required.
  • Provide information and reports to the Senior Leadership Team as requested.
  • Keep notes and records as appropriate during each shift for all patients and those under your supervision.

Key Skills & Experience:

  • The successful candidate will be registered to the NMC with no restrictions on practice and work with clear understanding of the NICE guidelines.
