This is an exciting job role working within the District Nursing service supporting patients efficiently within their own homes.

If yes, then this is the job for you. We are looking for a B3 Healthcare Assistant who believes in the value of quick intervention and nursing support to maintain peoples safety and independence at home.

The District Nursing service works as part of a wider integrated Directorate (iCares) working with people at home who have Nursing needs. This includes intensive nursing support and admission avoidance delivered by a multi-disciplinary team.

We work closely with our colleagues in Urgent Community Response, Locality teams and Virtual Wards, which means working closely with the wider Multi-disciplinary Team.

Our team work closely with patients to support and manage issues at home where possible and safe to do so. You will be supporting nurses and the wider MDT to achieve best possible patient centred outcomes.

Your line manager will be one of the nurses in the team and no one works in isolation. You will receive support and supervisions from your line manager who will be based within the team.

  • Provide the intensity healthcare support for people in the community.
  • To work closely and collaboratively with colleagues from the wider team, local authority and third sector.
  • To support the DN service in assessing and supporting patients in need of support in the community.
  • To bring value and compassion into everything you do.

Within this role you will be placed with one of our 7 District Nursing teams across Sandwell. You will receive relevant training and education alongside support from the Practice Education Team, Clinical Leads, Matrons and Team Leaders. You will be providing and supporting qualified nurses in performing nursing care such as diabetes management, wound care management and general nursing care to housebound patients. The role comes with autonomy and potential to grow as a practitioner.
