Clinical Lead – Complex Community Care

About the Role
We are seeking an experienced and clinically confident Clinical Lead to support and oversee complex care packages across the UK.
This is an excellent opportunity for a Registered Adult or Paediatric Nurse to provide clinical leadership and oversight for children and adults receiving complex care in their own homes.
You will be responsible for completing comprehensive clinical assessments and reviews, developing person-centred care plans, supporting care teams, and ensuring that staff have the appropriate skills, training and competencies to deliver safe, effective and high-quality care.
The successful candidate will work collaboratively with families, Case Managers, ICBs, Social Services, NHS and private hospitals, and wider multidisciplinary teams to ensure the effective mobilisation and ongoing management of complex care packages.
Key Responsibilities
- Complete comprehensive clinical assessments and ongoing clinical reviews.
- Develop, implement and regularly review person-centred care plans and risk assessments.
- Provide clinical oversight and support for children and adults receiving complex care at home.
- Provide clinical guidance and support to care teams at the point of care.
- Maintain high standards of safeguarding, clinical governance, clinical quality and risk management.
- Conduct clinical observations, spot checks, audits and competency assessments.
- Support, supervise, mentor and develop carers and support workers.
