About the Role
We are seeking an experienced and clinically confident Clinical Lead to support and oversee complex care packages across the UK.
This is an excellent opportunity for a Registered Adult or Paediatric Nurse to provide clinical leadership and oversight for children and adults receiving complex care in their own homes.
You will be responsible for completing comprehensive clinical assessments and reviews, developing person-centred care plans, supporting care teams, and ensuring that staff have the appropriate skills, training and competencies to deliver safe, effective and high-quality care.
The successful candidate will work collaboratively with families, Case Managers, ICBs, Social Services, NHS and private hospitals, and wider multidisciplinary teams to ensure the effective mobilisation and ongoing management of complex care packages.
Key Responsibilities
Complete comprehensive clinical assessments and ongoing clinical reviews.
Develop, implement and regularly review person-centred care plans and risk assessments.
Provide clinical oversight and support for children and adults receiving complex care at home.
Provide clinical guidance and support to care teams at the point of care.
Maintain high standards of safeguarding, clinical governance, clinical quality and risk management.
Conduct clinical observations, spot checks, audits and competency assessments.
Support, supervise, mentor and develop carers and support workers.
Deliver clinical training and assess staff competencies against required standards.
Support the mobilisation, implementation and ongoing management of complex care packages.
Work closely with families, Case Managers, MDTs and other external stakeholders.
Liaise effectively with ICBs, Social Services, NHS and private hospitals and other healthcare professionals.
Identify and manage clinical risks, escalating concerns appropriately.
Promote a culture of person-centred care, continuous improvement and clinical excellence.
Ensure care delivery remains compliant with relevant legislation, professional standards, policies and clinical governance requirements.
Maintain accurate and comprehensive clinical documentation and records.
About You
We are looking for a motivated and experienced nurse who is confident working autonomously while providing strong clinical leadership and support to care teams.
You will have:
Current registration as an Adult or Paediatric Nurse with an active NMC PIN.
Strong clinical experience supporting individuals with complex healthcare needs within a community or homecare setting.
Strong clinical knowledge and practical experience in areas such as tracheostomy care, PEG feeding, suctioning and airway management.
Experience supervising, supporting and developing care staff.
Experience delivering clinical training and completing competency assessments.
Experience completing detailed clinical assessments, care plans and risk assessments.
Strong knowledge of clinical governance, safeguarding and risk management.
Excellent communication, interpersonal and organisational skills.
The ability to build effective working relationships with families, healthcare professionals and multidisciplinary teams.
A confident and professional approach to providing clinical advice and guidance.
The ability to work independently, prioritise workload and manage multiple care packages.
A commitment to delivering safe, high-quality and person-centred care.
