Role Overview:
As a
Senior Social Worker
within the
Adult Locality Team
, you will manage complex cases, provide specialist advice, and support service delivery across the team.
You will work closely with the
Team Manager
and a range of internal and external partners to ensure high-quality, person-centred outcomes.
What you will do:
Lead and coordinate
Continuing Healthcare (CHC)
assessments and reviews, both face-to-face and virtually.
Manage a complex caseload from initial assessment through to resolution, ensuring timely and effective outcomes.
Arrange and review care packages and placements in partnership with health providers, commissioning teams, and external agencies.
Provide specialist advice on
CHC
,
Section 117 aftercare
, and
joint funding pathways
.
Maintain accurate, high-quality records and contribute to
data reporting
,
governance
, and
performance monitoring
.
Support service development initiatives and contribute to achieving team performance targets.
Assist with staff induction, mentoring, and training to promote continuous improvement.
