Are you ready to take the next step in your social work career? We are seeking an experienced Senior Social Worker to join the Adult Locality Team in Medway, Kent. Role Overview: As a Senior Social Worker within the Adult Locality Team, you will manage complex cases, provide specialist advice, and support service delivery across the team. You will work closely with the Team Manager and a range of internal and external partners to ensure high-quality, person-centred outcomes.
What you will do:- Lead and coordinate Continuing Healthcare (CHC) assessments and reviews, both face-to-face and virtually.
- Manage a complex caseload from initial assessment through to resolution, ensuring timely and effective outcomes.
- Arrange and review care packages and placements in partnership with health providers, commissioning teams, and external agencies.
- Provide specialist advice on CHC, Section 117 aftercare, and joint funding pathways.
- Maintain accurate, high-quality records and contribute to data reporting, governance, and performance monitoring.
- Support service development initiatives and contribute to achieving team performance targets.
- Assist with staff induction, mentoring, and training to promote continuous improvement.
- Ensure compliance with GDPR, safeguarding responsibilities, and organisational policies.
- Promote the wellbeing and safeguarding of adults at risk and contribute to corporate parenting responsibilities where applicable.
- Uphold the principles of the Equality Act 2010, Health and Safety legislation, and local authority statutory duties.
- Contribute to emergency planning responsibilities as required, including major incident responses.
- Degree in Social Work (BA / MA / DipSW).
- Registration with Social Work England.
- Substantial post-qualifying experience within Adult Social Care.
- Experience of complex case management, CHC processes, and multi-agency working.
- Ability to provide professional leadership and support to colleagues.
