What You Will Do
- Lead and coordinate Decision Support Tool (DST) assessments to determine eligibility for NHS Continuing Healthcare (CHC) funding.
- Manage complex and high-risk cases, including disputes and joint funding arrangements, providing professional and compassionate support throughout the process.
- Work closely with care providers and local authority partners to develop appropriate care packages, whether or not individuals meet CHC criteria.
- Undertake Section 117 reviews and contribute to service improvement initiatives to ensure high-quality, person-centred service delivery.
- Provide professional supervision, guidance, and support to a small team of social care staff, promoting best practice and continuous professional development.
- Liaise with a wide range of internal and external stakeholders to implement service strategies aligned with organisational values and objectives.
