What you will do:

  • Conduct holistic assessments of patients in community bed settings.
  • Develop and implement personalised rehabilitation plans to support patient recovery and transition home.
  • Collaborate with multidisciplinary teams, including occupational therapists, physiotherapists, and other healthcare professionals.
  • Support patients and families to understand and engage with rehabilitation goals.
  • Document and report progress, adjusting treatment plans as required.
  • Provide education and training to patients and families on maintaining rehabilitation objectives.
  • Ensure compliance with healthcare regulations and uphold high standards of patient care.

Requirements:

  • HCPC registration (mandatory)
  • Minimum 2 years experience in occupational therapy or related specialism
  • Car driver preferable (not essential)
