What you will do:
- Conduct holistic assessments of patients in community bed settings.
- Develop and implement personalised rehabilitation plans to support patient recovery and transition home.
- Collaborate with multidisciplinary teams, including occupational therapists, physiotherapists, and other healthcare professionals.
- Support patients and families to understand and engage with rehabilitation goals.
- Document and report progress, adjusting treatment plans as required.
- Provide education and training to patients and families on maintaining rehabilitation objectives.
- Ensure compliance with healthcare regulations and uphold high standards of patient care.
Requirements:
- HCPC registration (mandatory)
- Minimum 2 years experience in occupational therapy or related specialism
- Car driver preferable (not essential)
