What you will do
- Screen and prioritise complex referrals received through the Contact Centre to ensure immediate needs are met efficiently.
- Conduct person-centred, strength-based assessments to formulate effective support plans.
- Provide expert guidance to Enabling Independence Officers and Trusted Assessors to promote excellent service delivery.
- Work collaboratively within a multidisciplinary duty team to deliver comprehensive support to the community.
- Recommend and implement suitable equipment and adaptations, facilitating residents' independence.
- Complete proportional assessments, effectively signposting residents to the appropriate services.
- Demonstrate safe and effective use of recommended equipment to clients and their families.
- Assess eligibility and manage processes for major adaptations, including structural changes and lifts.
- Present cases to Quality Assurance Panels as required to maintain high standards of client care.
- Liaise efficiently with Housing, Social Care, and partner agencies, ensuring coordinated support for residents.
- Assist in finding suitable rehousing solutions for clients when necessary.
