What you will do
- Provide high-quality social work practice within the Transitions Team focusing on individuals moving to adult social care.
- Conduct assessments and support planning for service users with Learning Disabilities, Autism, and other needs to promote their independence.
- Collaborate effectively with a wide range of professionals to ensure comprehensive care and support for service users.
- Develop and maintain accurate and timely service user records and prepare detailed reports as required.
- Attend County Hall at least once a week (Tuesdays), ensuring a balanced and collaborative team environment.
- Advocate for service users\’ needs and their right to maintain control and make choices about their lives.
