What you will do

  • Provide high-quality social work practice within the Transitions Team focusing on individuals moving to adult social care.
  • Conduct assessments and support planning for service users with Learning Disabilities, Autism, and other needs to promote their independence.
  • Collaborate effectively with a wide range of professionals to ensure comprehensive care and support for service users.
  • Develop and maintain accurate and timely service user records and prepare detailed reports as required.
  • Attend County Hall at least once a week (Tuesdays), ensuring a balanced and collaborative team environment.
  • Advocate for service users\’ needs and their right to maintain control and make choices about their lives.
