Join us on an exciting journey as a Safeguarding Social Worker within the Adults Safeguarding Team in Ealing. For three months, you will immerse yourself full-time in a role that challenges and rewards, providing safeguarding expertise for adults and collaborating with families and professionals to ensure the best outcomes. What you will do:
* Conduct comprehensive strength-based assessments for adults, carers, and families, coordinating with other professionals to deliver specialist assessments.
* Identify and mitigate risks, ensuring robust safeguarding practices are in place at all times.
* Support and preventative work with individuals and families, assisting carers and families to maintain home care, including within the Ealing Shared Lives service.
* Engage with adults, their carers, and families through regular visits, providing exceptional support for those with complex needs.
* Lead efforts to arrange optimal care levels for adults unable to be cared for in the community, supporting less experienced social workers.
* Collaborate for the rehabilitation of adults in partnership with families and other key stakeholders, developing care plans that cater to individual needs.
* Facilitate communication between carers, families, and professionals, supporting adults in various placements.
* Lead safeguarding investigations and take decisive action to protect adults in need of care and control.
* Keep abreast of legislation, regulation, and best practices, using this knowledge to inform your work effectively.
* Maintain accurate client records and collaborate with other agencies to achieve exceptional outcomes for adults.
