Job Duties:
- Provide social care service and exemplar social work practice ensuring excellence of service across the department.
- Contribute to change management, concluding ECC's transformation and ensuring continuous improvement.
- Provide support and guidance to community mental health nurses and lead on key aspects of patient care.
- Lead and offer support to health colleagues in relation to S.117 needs and reviews, alongside completing Care Act Assessments and arranging funded care.
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant frontline experience in Adults Services
- Mental Health experience and knowledge of the Care Act
- An in-depth understanding of the financial implications of commissioned care packages.
- The ability to make clear decisions and limit potential risks associated with intervention.
- A driving licence and car - or the ability to meet the mobility requirements of the role through other means - is essential.
