Requirements of the Social Worker: * Degree or equivalent in social work * Current Social Work England registration * Significant frontline experience in Adults Services Main Suties: * Manage a caseload as allocated * Undertake assessments under the Care Act and other legislation, using a strength- based approach * Co-ordinate and monitor care packages and support (care) plans and undertake reviews to ascertain whether the desired outcomes are being achieved. This may include advocating on behalf of service users or carers * Work with individuals to assist them to assess risk and take decisions related to their ability to function in their community environment * Lead Safeguarding Adults investigations using the local multi-agency policy and procedure (Level 2 Social Workers only) * Promote safeguarding awareness with service users and multi-disciplinary colleagues.
