SOCIAL WORKER – Level 3 – Social Care Qualified in Swansea earning £38.76 per hour. We have an extraordinary temporary opportunity for a skilled Social Worker in Swansea, lasting 8 weeks. You will work approximately 38 hours per week as part of a vibrant and effective Hospital Assessment Team. What you will do:

  • Conduct comprehensive Mental Capacity Assessments to ensure patients' needs are accurately captured.
  • Lead Best Interest Meetings to facilitate safe and timely hospital discharges in line with individuals' care plans.
  • Collaborate closely with health board colleagues, therapists, families, and other external providers to deliver a seamless service centred around patient needs.
  • Undertake Continuing Health Care Assessments, advocating for the best welfare strategies for the patient's discharge.
  • Engage actively in multi-disciplinary team meetings to enhance patient outcomes and ensure alignment on discharge plans.
