What you will do
- Build strong, trusting relationships with young people, encouraging engagement and independence.
- Provide practical, emotional, and social support to young people transitioning from care to independent living.
- Conduct assessments of individual needs and contribute to Pathway Plans and support plans.
- Support young people to identify goals and develop action plans to achieve their aspirations.
- Provide advice and guidance on accommodation, education, employment, training, health, and wellbeing.
- Teach and promote essential independent living skills, including budgeting, cooking, and household management.
- Advocate for young people's rights and ensure their views and wishes are central to planning and decision-making.
- Identify and respond appropriately to safeguarding concerns and emerging risks.
- Maintain regular contact with young people, adapting support plans as circumstances change.
- Work collaboratively with social workers, education providers, housing services, health professionals, and partner agencies.
- Assist young people in securing accommodation and maintaining successful tenancies.
- Promote physical and emotional wellbeing through access to appropriate health and support services.
