What you will do
- Conduct assessments and reviews of social care needs for adults under the Care Act.
- Develop person-centred, strengths-based support plans.
- Undertake capacity assessments under the Mental Capacity Act.
- Prepare Social Circumstances Tribunal reports.
- Take on the role of a Social Supervisor for conditionally discharged s37/41 patients.
- Act as a Safeguarding Enquiry Officer, collaborate with Safeguarding Adults Managers.
- Participate in a duty system for screening referrals and handling safeguarding concerns.
