What you will do:

* Deliver high-quality interventions following AfC\’s assessment, planning, and intervention framework.

* Conduct comprehensive whole-family assessments, maintaining professional boundaries and establishing realistic goals.

* Coordinate and deliver targeted support for families, emphasising the well-being of children and young people in decisions.

* Lead in priority areas such as Domestic Violence, Parental Mental Health, Housing, or SEND.

* Identify and work with multi-agency partnerships to develop SMART plans in collaboration with families.

* Record all assessments and interventions meticulously on case management systems.

* Respond to significant events promptly and escalate matters appropriately to management.

* Sustain positive changes, increased independence and reduced risk factors for families.

* Promote a culture of partnership and innovation within the team and service.

* To thrive in this position, you must have a Degree or equivalent in Social Work, a current Social Work England registration, and substantial frontline experience in social services within the UK.
