Key Responsibilities
- Conduct assessments of individuals and families to identify care needs and develop effective care plans.
- Collaborate with external agencies and service providers to deliver holistic support.

- A full driving licence and access to a vehicle is advantageous.
- All employment gaps must be accounted for and verifiable.
- Degree in Social Work and current registration with Social Work England.
