What you will do:
- Assess individuals in their own homes following hospital discharge
- Support safe and timely hospital discharges through the Discharge to Assess pathway
- Conduct comprehensive functional assessments and activity analysis
- Recommend and prescribe necessary equipment, assistive technology, and minor adaptations
- Undertake and review moving and handling assessments
- Collaborate with multidisciplinary partners to develop personalised support plans
- Promote independence while minimising reliance on ongoing care when suitable
- Assist individuals, families, and carers during periods of change and recovery
- Maintain accurate and punctual case records
Requirements:
- Registration required: Yes, with the HCPC
- Car driver: Yes, due to travel needs across the community
