What you will do:
- Undertake initial Adult Social Care triage, assessment, and screening of referrals received by the service.
- Conduct strengths-based and person-centred assessments, identifying eligible needs under the Care Act 2014.
- Respond promptly to safeguarding concerns and contribute to Section 42 safeguarding enquiries in line with local and national procedures.
- Complete Mental Capacity Act assessments and contribute to Best Interest decision-making where required.
- Provide information, advice, signposting, and preventative interventions to reduce, delay or prevent the need for statutory services.
Person Specification:
Qualifications:
- Degree, Diploma, or equivalent qualification in Social Work.
- Successful completion of the Assessed and Supported Year in Employment (ASYE) or equivalent post-qualification experience.
Registration and Compliance:
- Current registration with Social Work England.
Experience:
- Post-qualified Adult Social Care experience within a UK Local Authority.
- Experience undertaking Care Act assessments, support planning, and safeguarding work.
