What you will do
- Conduct comprehensive assessments of service users presenting with substance misuse and associated vulnerabilities.
- Develop, implement, and review personalised recovery and resettlement plans to ensure continuity of care post-release.
- Coordinate access to vital resources such as treatment services, healthcare, housing support, employment programmes, and more.
- Maintain effective working relationships with important entities including prison healthcare teams, security departments, probation services, and partner agencies.
Person Specification
Qualifications:
- NVQ Level 3 or equivalent in Health and Social Care, Substance Misuse, Criminal Justice, or a related field.
- Evidence of continuing professional development in recovery, addiction, or offender rehabilitation.
- Desirable training in safeguarding adults and trauma-informed practice.
Registration and Compliance:
- Ability to obtain and maintain Prison Security Clearance.
Experience:
- Track record supporting individuals affected by substance misuse, offending behaviour, or complex social challenges.
- Experience working within criminal justice, prison, probation, community recovery, or related health and social care settings.
- Proven track record of collaborating with both statutory and voluntary sector organisations.
