What you will do:
- Provide senior clinical input within the Community Mental Health Team, delivering high-quality assessments, interventions, and care planning.
- Offer clinical advice, consultation, and professional challenge to Band 6 practitioners and the wider multidisciplinary team.
- Act as Named Worker for an agreed caseload, developing personalised care plans and overseeing effective care coordination.
- Chair or contribute to multidisciplinary discussions, daily huddles, complex case reviews, and clinical decision-making.
- Provide clinical supervision, guidance, and support to Band 6 practitioners and student staff.
- Work collaboratively with GPs, local authority colleagues, crisis services, and other partner agencies to promote effective care pathways.
- Contribute to clinical governance, including audits, quality assurance, risk management, and service improvement initiatives.
- Promote evidence-based practice and maintain high standards of clinical care across the service.
