What you will do:
- Provide comprehensive pharmaceutical care to patients on surgical wards, ensuring the safe and effective use of medication.
- Perform accurate medication history checks and provide counselling to patients regarding their prescribed medications.
- Collaborate closely with the multidisciplinary team to develop individualised care plans.
- Ensure accurate and efficient dispensing of medications to enhance patient outcomes.
- Maintain up-to-date and thorough documentation according to department protocols.
- Participate in ward rounds to provide pharmaceutical expertise and immediate therapeutic advice.
Requirements:
- HCPC registration.
- Minimum of 2 years’ experience working in surgical and medical adult wards.
- Strong knowledge of pharmaceutical care within acute hospital settings.
- Excellent communication and multidisciplinary team-working skills.
- Driving licence preferred but not essential.
