What you will do
- Provide urgent physiotherapy assessments and interventions to support hospital admission avoidance.
- Respond promptly to referrals for patients experiencing acute mobility, functional, or rehabilitation needs.
- Undertake comprehensive assessments of mobility, balance, strength, transfers, and falls risk within community settings.
- Deliver timely rehabilitation programmes aimed at maximising independence and preventing further deterioration.
- Develop and implement person-centred treatment plans to ensure patients can remain safe at home.
- Work collaboratively with multidisciplinary teams to provide coordinated crisis response services.
- Support early supported discharge programmes and contribute to reducing avoidable hospital readmissions.
